What a new patient information form needs, and what it does not

The useful core is short. Full name, date of birth and preferred name. Address, phone and email, with the patient's preferred contact method and whether it is acceptable to leave a voicemail. Emergency contact with relationship and number. Insurance carrier, member and group identifiers, and the subscriber where that is not the patient. Employer only if your practice genuinely needs it, which most do not any more. What is missing is deliberate: no Social Security number by default, because the overwhelming majority of practices do not need one to register a patient and collecting it by habit creates a liability with no matching benefit. No medical history, no consent language, no records release. If you need those, they are proper documents in their own right rather than an afterthought at the bottom of page one.

Where the fuller forms live

Keeping this form short only works if the other jobs have somewhere to go, so they do. Clinical background belongs on a medical history form, which has room for medications, allergies and past conditions and a repeating structure that a registration page cannot sensibly carry. A patient's whole first-visit packet — registration plus history plus policies together — is a general intake form. Authorising the release of records to another provider or a family member is a HIPAA release, a legally loaded document that should never be reduced to a tick box on a registration page.
