Patient name, date of birth, the address where care will be provided and how to reach them, plus a contact for a family member or caregiver, which agencies need more often than they need the patient. Insurance or payer details where your workflow includes them. The referral source and date. Then the ordering or certifying provider in full — name, credentials, NPI, practice, phone, fax and signature — because the agency will need to reach them and, for Medicare cases, because certification is theirs to give. The clinical entry: the primary diagnosis and relevant secondary conditions, recent hospitalisation or surgery with dates, current medications where you can share them, and functional and mobility status as the referrer observed it. Then the requested disciplines, and a supporting-document checklist covering the discharge summary, medication list, recent notes and face-to-face documentation, marked as attached, following or available on request.

This is the section that decides how quickly a case opens.
